Park Avenue Walk In Clinic was established by Dr. Rezai, in 2005 in order to provide affordable and easily accessible medical services to bay area community.
Dr. Rezai is a USCIS certified physican, and he provides complete physical exam and vaccinations required by USCIS at a very affordable price.
We provide the most affordable, reliable and pleasant medical services to our patients. Having trilingual (Spanish, English, Persian) Clinic staff and medical team makes our medical services very convenient. With our excellent office visit fees there is no need to have medical insurance to pay for your office visit. We guaranty same day appointment. If not able to make
a same day appointment we will provide next day appointment with 50% off on our regular office visit fee to show our commitment to our patients. ( Just for patients with no medical insurance)
About Dr. Rezai 
Dr. Shahin Rezai is certified by American Academy of family Practice Board. He is also certified in Cosmetic Botox Injection for Facial Wrinkle treatment by Aesthetic Health Enhancement Institute.
Dr. Shahin Rezai completed the training in family practice residency at UCSF. He also completed one year of training in pediatrics residency at Mount Sinai School of medicine. He graduated from University of California San Francisco in 2005. He was granted two awards for 100% continuity medical care during the family practice residency training.
